Do you have at least 5 blank visa pages? If NOT, click here.

Is your passport in good condition? If NOT, click here.

Is your name still the same? If NOT, click here.

Will your green card be valid at the time of travel? If NOT, notify the missions office.

FOR LEBANON ONLY: Do you have an Israeli visa stamp or border crossing stamp in your passport? If so, you will need to apply for a limited crossing stamp in your passport? If so, notify the missions office.
